Copy of any name change document(s) from IRS and confirmation letter from IRS (not organization heads and the address noted on the most recent letter from the IRS should be the either the current mailing address or physical address of organization)
Organizations under a Group Ruling must provide a copy of the IRS Group Letter.
All organizations under a group ruling must also provide either a letter from the Group holder with a live signature dated during the year funds are requested showing recognition by the group holder with their individual Tax Identification Number shown and name of the entity under the Group named as recognized by the group holder. The letter from the group holder must be dated within the current year (year seeking to receive funding). This must be included with the rest of the documentation to be considered.
Or if the Group holder publishes a directory annually of all entities recognized by them to include the name, location and Tax Identification Number you must provide a printed copy of the cover and a printed copy of the page the subordinate organization is listed and enclose with the supporting documents. These copies must be from the directory for the year support is sought. Religious Organizations with Group Ruling letter 501(c)(3) from the IRS need to provide the group ruling letter, the cover of the current year’s directory from the Diocese/Archdiocese/District, et al and a copy of the page from the directory acknowledging your recognition. (For charities under the U.S. Catholic Conference of Bishops, the letter has been changed and Tax Identification Number has also changed effective May 2016. If you do not have a current copy of this letter, please download a copy from the U.S. Catholic Conference of Bishops website for your use and records.)
The most recent 990-PF for those charities required to file and for those that file a 990-T and not a 990-PF the most recent available at time of submission (this is required) or any other form of Tax Document. The document must be complete, include all schedules with your submission.
Most recent Audited Financial Statements (should you have an audit performed)
Include a detailed budget if requesting for a specific program or project
Include a detailed organizational operating budget (all expenses of the organization with the exception of a capital campaign or endowment). All organizations must provide a detailed operating budget. Larger organizations providing an “extremely condensed budget” will be contacted to provide a fully detailed line item budget as all others are required to provide.
The required application form must be completed online and emailed to Patricia Stilley at pstilley@ictft.org. The required application form will only be accepted via email. You are welcome to include a printed copy of the completed form in with your packet but understand it will not replace the copy we require by email. It must also be received prior to January 31 st by 5 pm to be considered.
A list of current board of directors and must include their addresses and phone numbers (they may be either office or home-we do not make it a policy to call any board members unless there is a question that must be posed directly to them).
Major donor list for year of request (to include the name of funder, approved grant and amount as well as any requests that are under consideration or those requests you will be submitting during the current year).
509 (a) (3) Letter (found under Letter Template tab on site) If your organization falls under category 509(a)(3). To determine whether or not your organization falls under this classification refer to your post 1969 Letter of Determination from the Internal Revenue Service, second paragraph. Most organizations are classified as 509(a)(1), however if your organization is a 509(a)(3) you must complete the template letter as described and include it with your supporting documents.
Required Related Party Questionnaire. Relationship between your organization’s staff and Board in relation with the Isla Carroll Turner Friendship Trust Board. It must be completed and enclosed with the rest of the supporting documents that are mailed to or hand delivered to our offices prior to the deadline. The current list of our Board Members may be found on our website Board List tab.
You are also welcome to enclose a more detailed description of the history, mission, services and/or project with your supporting documents. We ask that it be kept to approximately one to four (1-4) pages in length and please do not reference this description when completing the online form as the board will not have this included in their packet. It will be kept in the office with the rest of the proposal to reference should any board member ask for additional information or by our office staff to expand our knowledge of the organization in preparation for the meeting.

ALL documents including the application form must be received NO LATER than January 31st by 5:00 p.m. to be considered. Proposals will be accepted as early as August prior to the January 31st deadline. Late documents or proposals will not be accepted.
